Dental Implants and Immediate Load -At Dr. Gradwell's office the use of implants is very common. We see dentures as a concept of the past. At this point in time the art of implants has been mastered. There is no need to deal with the many hassles caused by dentures. Dentures can cause sores and it may be almost impossible sometimes to eat with them. With implants you can have teeth that look real and funtion perfectly.
Another advantage of implants is that luckily the process is not very challenging and is almost totally painfree. You will come in for your first appointment and your case will be diagnosed. Once Dr. Gradwell is ready, a titanium root will be placed into your mouth. This root will bond to your jaw bone allowing for maximum stability like any other tooth. Over a period of 3-6 months this titanium root will integrate with your jaw structure. Often times, this process can be done in a single day. That would be called "immediate load" and will be discussed below. As for a normal implant, once the root is done bonding, it will be uncovered and an abutment will be placed on it. Once the abutment is on, your new tooth will then be placed. If this new tooth is stable then you will leave that day with a tooth. For 90 days though you will be limited to soft foods. Six months after you get the tooth you will need to come back to get a permanemt crown that will look exactly like your tooth. You will never know it was there after the healing period is finished. Implants are a top achievement in Dental Science.
Immediate Load:
Another option available with Dr. Gradwell is an "immediate load" or same day teeth. With this process you will come in for your first appointment and you will leave that day with your tooth. With "immediate load" the titanium root will be bonded that day to your jaw bone using space age dental materials. The abutment is placed and then the tooth. The 90 rule still applies and you will still have to come back in 6 months for your permanent crown. But now after one appointment you can fill any uncitely gaps immediatly or take out painful, unhealthy teeth, and replace them in only 90 minutes. -Periodontal disease, also known as "gum disease", is a chronic bacterial infection that damages the gums and bone supporting the teeth. It is a very serious disease that can result in tooth loss, but it is generally painless, so you might not even know you have it. There are different stages of periodontal disease - Gingivitis, Beginning Periodontitis, Moderate Periodontitis and Advanced Periodontitis. Depending on the severity of the case type, it will be treated with scaling and root planing or osseous surgery.

Scaling and Root Planing
-Periodontal scaling is a procedure involving instrumentation of the crown and root surfaces of the teeth to remove plaque, calculus, and stains from these surfaces. It is performed on patients with periodontal disease. Periodontal root planing is a definitive, meticulous procedure designed to remove cementum and/or dentin that is rough, may be permeated by calculus, or contaminated with toxins or microorganisms. When done in a thorough fashion, some unavoidable soft tissue removal occurs. It is recognized that debriding the root surface is a critical element in establishing periodontal health.

Crown Lengthening
-Caries, fracture of a tooth, external or internal root resorption, uneven occlusal planes or excessive occlusal or incisal wear may result in insufficient clinical crown length or tooth structure to support a restoration without impinging upon the biologic width of the tooth. Placement of a restoration without adequate biologic width may result in inflammation and loss of support. In order to maintain periodontal health there should be adequate exposure of sound tooth structure for the restoration of the tooth.
A crown lengthening procedure is often necessary to obtain adequate sound tooth structure. Clinical crown lengthening is performed in a healthy periodontal environment, in contrast to osseous surgery, which is performed to treat periodontal osseous defects.

Subepithelial Connective Tissue Graft
-The subepithelial connective tissue graft procedure is performed to create or augment gingiva, to obtain root coverage to eliminate sensitivity and to prevent root caries or to eliminate frenum pull.
The tissue is taken from the donor site for a connective tissue graft and is placed in the recipient site. The donor site is sutured closed resulting in a small wound. The tissue is then secured at the recipient site with sutures.
The degree of difficulty and the expertise required to perform a subepithelial connective tissue graft is significantly greater than for a free soft tissue graft procedure. The subepithelial connective tissue graft procedure results in two sutured closed wound sites, while the free soft tissue graft results in two open exposed wounds.

-Soft tissue grafts are surgical procedures designed to increase or restore the zone of keratinized tissue. These procedures are performed to reestablish health by stopping progressive recession and to improve esthetics.
